I was told by my pediatric ENT (Ear Nose and Throat) doctor that a child needs their tonsils until age 2. After age 2, the tonsils no longer serve a purpose; so the signs of a toddler needing tonsils out would be as follows:

Not sleeping well, waking a lot (sleep apnea)
Look enlarged

Usually the first tell tale sign of a toddler needing their tonsils removed would be the sleep apnea symptoms such as sleep walking, night terrors and staying in bed all night but not seeming as if they even slept. Snoring is a big one too, the ENT said a child should never snore, if snoring something is up.
