Baby Being Able to Hold Head Up On Tummy Time

My son has been holding his head up during tummy time (not too long and barely high, but still did so). Recently, he has stopped this. He turns his head to the side and starts sucking his fist and refuses to respond to my voice or toys. He has a physical therapist that comes into our home once a week to work with him, mostly do to his hands (he will be needing surgery to correct them), and I work with him the remaining days. I know that he probably doesn't push himself up do to his hands, but he will not hold his head up anymore. He is four months old. Should I be concerned?

I am going to ask his PT tomorrow, but I was wondering if any other mothers experienced extremely delayed milestones. If so, is it a cause for concern? I am hearing from family members on both sides that it is, so I am worried. To me, I think he will get there on his own time. Am I wrong?

*Sorry for the typos. He became fussy and was squirming in my arms.

      Thank you. :)
He is above verbally, socially, and mentally, it is just his physical milestones. I work with him, daily. I just don't understand why he is still near a 2 month old stand point, and he is four months old. I just feel lost as what to do. Thanks again, Sheila.

Just keep working with him and doing tummy time too. He'll get there don't worry to much, your doctor might have some suggestions too. All babies learn at different rates.
